REFERRING to Bob Tavani House for Medical Respite

• Age 18 or over 
• Has a PCP in Duluth or is willing to establish PCP at Duluth Family Medicine Clinic (DFMC) or another local primary care clinic
• Does not require current inpatient drug/alcohol treatment program
• Experiencing or at risk of homelessness
• Is deemed by provider to be a poor candidate for discharge to self-care in street or shelter setting due to high risk of readmission, unstable chronic disease, incomplete recovery from acute illness, dependence on stored / scheduled medications, etc.
• Has potential for improvement and discharge within approximately 6 weeks 
• Does not require more than minimal assistance with activities of daily living
• Does not otherwise qualify for short term rehab, long term care, or acute rehab
• Case worker has screened patient, and no viable and immediate housing option has been identified
•  Capable of maintaining a safe and stable living environment with the help of live-in staff or volunteers
• Interest in coming to the Medical Respite House
• Signs agreements to follow program rules and participate in own care
• Case reviewed and cleared by DFMC medical respite team

1. Case workers identify patient who may be good candidate for BTH
2. Case workers or the potential guest call the house at 218-464-1772 or email info@bthrespite.org to give patient information
3. If there is an opening or an anticipated opening , BTH staff text/call DFMC residents (inpatients) or designated BTH staff (outpatients) with patient info
4. Within 24-48 hours, DFMC residents/BTH staff screen patient and communicate with case worker if patient is accepted
5. Patient will be transported by BTH staff or voucher to BTH
